Enrollment Information and Forms

  1. Proof of Residence (must be dated within 30 days of enrollment): Utility Bill in parent/guardian’s legal name (Gas, Water, Electric only.)Note that phone bills, driver’s licenses as proof of address, or other mail are NOT accepted
  1. Parent/Guardian Identification (photo ID) State Identification (Valid Identification Card, Driver’s License, Consulate I.D.)Court Order establishing parent/guardian/educational rights (if applicable)
  1. Student Proof of Age and Identity (Original Birth Certificate, Baptismal Certificate)
  1. Immunization Record/Card
  1. Report Card from last school attended (if not an LAUSD school)
  1. IEP or 504 Plan (if applicable)
  1. Current Court Orders (if applicable)
